Overview

The Dashboard is part of the House of Care resources to support commissioning of health and person centred care for people with long term conditions (LTCs). It provides a view of multi-morbidity, quality of care and economic activity at CCG or Local Authority level.  It can support planning for commissioning by providing comparative information on risk areas and care outcomes.
