NHS Code4Health Application Form

Overview

A group/individual can apply to form a new community or be accredited to supply services to existing communities.

Communities are required to do the following:

• Create a core leadership team
• Putting up a website that includes a Code of Conduct
• Host quarterly NHS Code4Health events

You'll also be able to connect with other communities through our digital channels to learn how they work and to share your experiences with them.

Accredited providers are asked to provide details of how the services offered enhance the NHS Code4Health programme.
