Thank you for taking the time to read the consultation document. By clicking the link below you will be directed to a series of 10 questions which should take you roughly 15 minutes to complete. The questions will collect your views on the guidance and will make sure that it meets your needs. 
Responses to the survey are needed by Monday 30 May.

Due to the relatively condensed period between Royal Assent of the Health and Care Act 2022 and implementation on 1st July 2022, the consultation period is taking place over a relatively short period of time. However, the current draft of the guidance was developed in partnership with a group of partners who have experience of engaging, working with and representing people and communities. Therefore, we think this is appropriate under the circumstances.

All of the feedback is important and will be used to shape the final version of the guidance. Your responses to this survey will be anonymous. 

The consultation survey results and how we have considered them will be collated into a report that will be published on our website. The feedback is collated in a way that is non-identifiable.
