GIRFT CYA Diabetes Provider Questionnaire

Overview

NHS England's Getting It Right First Time (GIRFT) Team are rolling out a new workstream 'Children and Young People Diabetes' 

The workstream will bring together key metrics on diabetes care in 0-25 year olds , providing benchmarked and actionable insights. Whilst most of the data needed is already available, some important data is not. This questionnaire has been developed to plug those gaps, and quantify provider delivery at both Trust and Sysyem level in terms of resources including workforce. The questionnaire is spilt into two sections. The first section is for Trusts to fill out who provide a Paediatric Diabetes Service. The second section is for Trusts to fill out if they provide diabetes care to young adults (this may be a separate service or part of adult diabetes service). A separate questionnaire has also been sent for your local ICB to fill out.   The questionnaire should take approx 10-15 minutes to complete.
