This page is for the 2014/15 Community Pharmacy Contractual Framework National Audit on Emergency supply of medicines.  From 6 April 2015, community pharmacies that have undertaken the audit during period 1 (23 March – 5 April 2015) will be able to submit their collated results to NHS England. 

Please use the collated answers from the individual patient requests for emergency supplies, which you have collated on Form B, to fill in the following form.
