South East Special Care Dental Services Patient Survey - 1

Overview

We would like your views on NHS dental services which are provided for people with additional needs, for example if you are very anxious about visiting the dentist or if you have a learning or physical disability which makes it hard to visit a general dentist.

These services are sometimes known as Community Dental Services. We would like your views to help us to see if there are any changes we can make to improve them for you.

This survey can be completed by a patient themselves or by a parent or carer on behalf of a patient.
