Feedback form on future of Saltash Road Surgery, Plymouth
Overview
Short-term arrangements are in place to keep Saltash Road Surgery open until 31 March 2017, following Dr Robert Gardner's resignation. NHS England now needs to decide on the surgery's longer-term future.
Why your views matter
In our letter to patients, we set out why we thought the most-likely, practicable solution was for the surgery to close and for patients to register with another GP. However, no decisions will be taken until we have asked patients what impact this would have on them and looked at any other viable options. This survey form is your chance as a patient to feed back your views.
What happens next
Once we have heard and considered your views, we will be able to take our decision. This should be in late October 2016. After that, we will write to all patients about the decision.
