Survey on GP services at Mevagissey Surgery

Overview

As joint commissioners, NHS England and NHS Improvement and NHS Kernow Clinical Commissioning Group are looking at alternative options for patients of Mevagissey Surgery.

We are asking for valuable feedback from patients to understand how this could affect you and what impacts there may be in terms of accessibility, travel and range of services provided. 

Please answer as many questions as you can, as the more information we can collect, the better our understanding of what is needed for the future. For that reason, the survey is also open to people in the area who are not patients at Mevagissey Surgery. 

The deadline for responses is Wednesday 5 June 2019.
