GP Services in Plymouth

Survey on GP services at Ocean Health Group; Stirling Road, Collings Park and Chard Road Surgeries.

NHS England is looking at the long-term future of GP services for people currently registered with Ocean Health Group at Stirling Road, Collings Park and Chard Road Surgeries.

While a temporary agreement is in place to secure continuity of care for patients in the immediate future, following contract coming to an end in March 2018, this survey is about your views to improve services from April 2018.

Please answer as many questions as you can, as the more information we can collect, the better our understanding of what is needed. For that reason, the survey is also open to people in the area who are not patients of Ocean Health Group at Stirling Road, Collings Park and Chard Road.
