Overview

Person-centred care for long term conditions:  Our Declaration is a two page document NHS England and its partners have put together since April this year.  The declaration will spread a message and gather support around  how person-centred care can improve  experience and participation for people with long term conditions and their carers.  We consulted and have buy-in from a wide range of people and organisations,  including carers and communities, the public, health and social care professionals, policy makers, charities and other interest groups. 

Data protection advice: signing up to the declaration is anonymous unless you choose to leave your email and other contact details.  NHS England values your comments. We may use them in publications for our work on long term conditions but they will not be published with your name unless you agree to this.

Why your views matter

We’d like as many people to get involved by signing up to ‘Our Declaration’ and if they wish,  tell us  more about what they are doing to improve person-centred care. We are using the information we collect as a way of gathering support. We will keep an email contact list for people who want to stay involved.
