Survey on GP services at the Mayflower surgeries in Plymouth

Overview

NHS England is looking at the long-term future of GP services for people currently registered with Ernesettle, Mount Gould, Trelawny, Chard Road, Stirling Road and Collins Park surgeries.

All these surgeries – collectively known as Mayflower – are currently run by Access Health Care.

While a temporary contract agreement is in place to secure continuity of care until the end of March 2020, this survey is about the potential shape of services for the longer term.

Please answer as many questions as you can, as the more information we can collect, the better our understanding of what is needed. For that reason, the survey is also open to people in the area who are not patients at the Mayflower surgeries. If you completed a similar survey for the Ernesettle practice in 2016, please note that some of the questions have changed.
