Survey on NHS Domiciliary Dental Services in Derbyshire and Nottinghamshire

Overview

The contracts to provide domiciliary dental services in the Derbyshire and Nottinghamshire are due to expire at the end of March 2020. NHS England and NHS Improvement - Midlands is planning to commission new contracts to continue provision of this service and would like to ask patients and other service users who access this service to give their feedback on the current services, to ensure that the domiciliary dental service provision will meet the needs of the local population it serves. 

The survey will only take a few minutes to complete and can be done anonymously.
