Overview

NHS England and GPs in St Austell are working together to improve access and continuity of care for patients.They are also exploring ways to integrate primary care with other services in the town.

These proposed changes, which NHS England believes offer an exciting and sustainable model for the future, are being made possible by the merger of three of the GP practices in St Austell.

The practices involved are Park Medical Centre, Woodland Road Surgery and Wheal Northey Surgery. Patients at Polkyth Surgery are being asked separately whether they would like to be part of the merger or to keep the practice separate.

Benefits of joint working could include:

  • Extended hours services, including during evenings and at weekends, at an urgent care centre on one site
  • Improved continuity of care for those with complex needs or more serious and/or long term health problems
  • In the longer term, more services locally and integrating care through close working with other providers such as Peninsula Community Health


As someone who lives in the St Austell area, we are asking you to complete this survey to help shape the future provision, delivery and access to local primary care services. You don’t have to be registered with any of the current practices, including Polkyth Surgery, to have your say – the more people in and around St Austell who take part, the better.

Your responses will help ensure that primary care services meet the health needs of local people, as the information provided by the survey will be used to plan and evaluate services.
