GIRFT Cardiothoracic Surgery Questionnaire (Revisits)

Overview

GIRFT are launching the revisit programme for Cardiothoracic Surgery, following the first round of deep dive visits and publication of the national report in 2018. 

We will produce a refresh datapack for your unit and aim to revisit you in early 2021. 

The information provided in this questionnaire will feed into the datapack for your unit and we are very grateful for your input. 

The questionnaire is divided into three main areas: overarching information, cardiac surgery, and thoracic surgery. The questionnaire can be saved and reopened if needed.
