Pilot - GIRFT Neurology: Self-Assessment Toolkit

Overview

Dear Colleague,

Thank you for taking the time to complete this self-assessment questionnaire to help us better understand the shape of neurological services and care delivery across England.

This questionnaire is divided into eight sections. Free text answers are kept to a minimum to allow for better analysis, but we understand you may want to give additional information for a particular answer. There is a box to do this under ‘Additional Information’ at the end of the questionnaire. There is also a ‘save and come back later’ function, so you don’t have to complete this in one sitting.

Questions not marked specifically as (SNC) or (DGH) are for both Specialised Neurology Centres and District General Hospitals to answer.

We require only one response per Trust. Also, we would suggest the service’s clinical lead and operational lead are best placed to complete this together.
