GIRFT Ophthalmology Revisit Questionnaire 2020

Overview

In 2017 to 2018, your engagement with the GIRFT process was vital to enable us to identify the challenges we face delivering ophthalmology services, potential areas of unwarranted variation and examples of innovative and great practice. The GIRFT ophthalmology team would like to invite you to contribute to the second round of data collection before conducting revisits beginning in Spring 2020.

Results from this questionnaire will be used both to support these revisits, and to provide supporting evidence for a publication to follow up on progress since the original deep dive visits and the GIRFT Ophthalmology national report (published December 2019). In this latter document, no values for identifiable Trusts will be displayed.

Information Governance statement

The questionnaire will not collect patient identifiable information. The results from the questionnaire will be stored within the secure IT environment managed by NHS Improvement. Data will be accessed only by a small number of GIRFT staff who have been given authorised access by NHS Improvement, and only for the purpose of implementation of the GIRFT programme. Data will be deleted once it is no longer required for the stated purpose.
