This questionnaire is for the GIRFT Ophthalmology data metrics collection.

The collection is for the period Q3 2023/24. Responses are due back by 29th March 2024.

There should be one submission per trust. The data from your submission will be benchmarked and uploaded to the Model Health System (MHS) so please try to ensure the data is as robust as possible.
