GIRFT Oral and Maxillofacial Surgery - Lab Questionnaire

Overview

The information entered into this questionnaire will serve as a follow-up to the information provided from the first round of Oral and Maxillofacial (OMFS) deep dives to fully map OMFS laboratories across the country and the services provided.  Some of the questions will be about clinical data so it would be useful to have this to hand as you fill in the questionnaire. Please only include NHS data (not private).

This questionnaire MUST be completed by a Maxillofacial Prosthetic Laboratory Service Manager working in the trust's maxillofacial laboratory.

This questionnaire should take no longer than 20-30 minutes to complete (depending on if you have the data readily available). You will be provided with a PDF to help you to collate your answers as some questions may require research.

• Income
• Implants and models (you may need to source the answers for this section from the Clinical Director or manager for the service. These questions are related to current practice). It covers 3D planning, cutting guides and pre-formed plates for Head and Neck Cancer and Orthognathic Surgery.
