This questionnaire is for the GIRFT Ophthalmology data metrics collection.

The collection is for the period Q4 2025/26. Responses are due back before Friday 26th June 2026.

There should be one submission per service. The data from your submission will be benchmarked and uploaded to the Model Health System (MHS) so please try to ensure the data is as robust as possible.
