Super September: Outpatients staff feedback survey

Overview

Super September is an initiative to support elective recovery and empower locally driven change in as many acute providers as possible to accelerate current and new recovery initiatives, including by adapting routine follow-up arrangements during the period of Super September. It is important for those staff involved in Super September to have the opportunity to share and feedback on their experiences.

This survey is designed to capture the options and comments of those involved to assess the success of the initiative and how it can be improved in the future.
