Survey for GOS contractors on provision of services for seldom heard groups

Overview

NHS England – South (Wessex) Local Eye Health Network has set up a subgroup to look at how access to sight tests can be improved for people in seldom heard groups.

Please can you answer the following questions: To help the group understand if these people are being seen and if there are any factors that impact on seeing the people; what support would be helpful to you, your optometry team and for providers of domiciliary services.

This survey is designed to be completed by contractors, performers, managers and dispensing opticians of high street premises and domiciliary providers.
