GP services at Phoenix Surgery, Camborne

Overview

NHS England is looking at the long-term future of GP services for people currently registered with Phoenix Surgery. 

We are asking for valuable feedback from patients to understand what potential impact the closure of Phoenix Surgery may have in terms of accessibility, travel and range of services provided. 

Please answer as many questions as you can, as the more information we can collect, the better our understanding of what is needed. For that reason, the survey is also open to people in the area who are not patients at Phoenix Surgery.

If you can complete the survey online, that will save the NHS money and make analysis easier. 

If you would prefer to complete a paper version, copies are available to collect from Phoenix Surgery. Once completed, please mark it clearly and return your survey to the surgery for collation.
